Withdrawal Process

Families who wish to withdraw their student from Eagle View Middle School should complete the following:

  • Complete the withdrawal form below and include as much information as possible (new address and new school). Email the completed form to Mrs. Hendrix or drop off at the EV Main Office.

  • If moving within the United States, the new school will need to contact Eagle View Middle School with their release forms, this will allow Eagle View to withdraw the student and send appropriate records to the new school.

  • If moving outside of the country, please be sure to complete the withdrawal form with notation of the international move. Note the country moving and as much detail as is known.

On or before your student's last day at Eagle View:

  1. Return laptop and charger to the EV Counseling Center or EV8 Office (remember to have your student bring their charger)

  2. Return borrowed books to teachers

  3. Return borrowed library books

  4. If necessary, contact the school nurse of medication pick up needs to be coordinated
